- In a medium saucepan over medium-low heat, melt the butter completely.
- Add the heavy cream and bring to a gentle simmer — light bubbling around the edges, not a full boil. Cook, whisking often, for about 5 minutes until the cream begins to thicken slightly.
- Add the minced garlic and cook for exactly 1 minute, whisking constantly until fragrant.
- Add the grated Parmesan in 2–3 additions, whisking constantly after each until fully melted before adding more. This prevents clumping and ensures a silky-smooth sauce.
- Season generously with salt and freshly cracked pepper. Add nutmeg if using, starting with 1/4 teaspoon.
- If sauce is too thick, add a splash of cream or reserved pasta water and whisk to combine. Toss immediately with hot cooked pasta and serve right away.
